– Foucault skrev denne boken mens han underviste i fransk på universitetet i Uppsala, Sverige. Denne boken sees ofte på som en under-del av Foucaults overordnede verk; kunnskapens arkeologi. I boken forsøker Foucault å finne 'the turning point' mellom 'gammel' medisinsk lære, og den nye strukturerte læren. Han finner den i årene rundt starten av den franske revolusjonen (slutten av 1700 tallet). Skiftet, sier Foucault, består ikke hovedsaklig i endring i medisinsk praksis, men i endringen i semantikken, altså språket og betegnelsen av sykdommer. Den industrielle revolusjon, og økningen av bysamfunn utover 1700 tallet medførte behov for en struktur fordi flere måtte pleies. I tillegg hadde økningen av epidemier gjort behovet for forskning større. Foucault sitt poeng er imidlertid også at den opprinnelige ideen om en fri, og liberalisert utøving av legeyrket (hvem som helst skulle kunne bli lege), møtte side-effekter i form av økende antall sjarlataner, revolusjon førte til at frivillige leger gikk i krigen, og ble drept, noe som økte behovet sterkt for ytterligere skulering. Boken er relativt tung for en vanlig leser, men for trenede filosofer er den mer nyttig.
